DSHS Requirement for Reporting Building Leadership Changes

Skilled nursing and assisted living providers are responsible for reporting building leadership changes to DSHS.

For skilled nursing providers, WAC 388-97-1640 details the requirement for reporting changes to DSHS and to each resident each time there is a loss or a change in the nursing home’s administrator or director of nursing services at the time the loss or change occurs. For SNFs, this form is used to notify DSHS.

Assisted living licensees must notify DSHS within ten calendar days of the effective date of a change in the assisted living facility administrator, including full name and effective date of change, as detailed in WAC 388-78A-2570. Use the Assisted Living Change Report to notify DSHS.
