NWHRN Regional 2024 HVA Survey
As part of Northwest Healthcare Response Network (NWHRN)’s ongoing efforts to provide a well-informed Healthcare Coalition Hazard Vulnerability Assessment (HVA), NWHRN kindly requests the most appropriate person at your facility to please complete the following survey. In order to gather hazard-specific risk assessment data, respondents will be asked to provide the number of times your organization received and/or distributed alerts for specific hazard types, and the number of times your organization activated for specific hazard types during the 2024 calendar year. The survey is linked below and a PDF is also attached. Please complete one survey per facility by 5pm on February 15, 2025.
