SA2 ’25 Preliminary CMID Reports Are Available

The second Semi-Annual 2025 (SA2 ’25) Preliminary Case Mix Index Detail (CMID) Report is available.

The SA2 ’25 Preliminary CMID Report includes MDS assessments from 04/01/2025 to 09/30/2025 were transmitted to CMS/iQIES by 10/13/2025.

The scheduled summary is available at the Case Mix webpage for ALTSA.

Any assessments or modifications must be submitted by 11/14/2025 in order to be included in the SA2 ’25 Final CMID Report due out the week of 11/17/2025.

Confidentiality

When emailing the [email protected], please do not list the resident name or other identifying information unless you are using a Secure Email. Instead, please include only the “Resident ID” number found in the column on the right of the resident name on the CMID report.  We are able to review your inquiries as we have access to your CMID reports.  If we need to send you detailed information about a specific resident(s), we will send the information via Secure Email and you will need to log-in with a password to retrieve it.
