2nd Quarter Preliminary CMID Reports Coming Soon

The Q2 ’26 Preliminary CMID Report includes MDS assessments from 04/01/2026 to 06/30/2026 that are transmitted to CMS/iQIES by 07/09/2026.

This report will be emailed to the address authorized by your facility as of 07/09/2026.

Please transmit assessments or corrections by end of business day on July 9, 2026. Submissions will be included in the SA2 ’26 Final CMID Report in the week of November 9, 2026.

If you have questions about defaults, excluded residents, corrections, modifications, an inactivation, or need a copy of the CMID report please contact [email protected]

Confidentiality

When emailing the [email protected], please do not list the resident name or other identifying information unless you are using a Secure Email. Instead, please include only the “Resident ID” number found in the column on the right of the resident name on the CMID report.  We are able to review your inquiries as we have access to your CMID reports.  If we need to send you detailed information about a specific resident(s), we will send the information via Secure Email and you will need to log-in with a password to retrieve it.
