Dementia Action Collaborative Survey Now Open

The Dementia Action Collaborative (DAC) is seeking input from providers about barriers and challenges of screening, diagnosing, and caring for people living with dementia. Your feedback will help guide them toward a health care system that meets the needs of people affected by dementia.

You can help by completing this 15-minute Provider Survey and sharing the survey link with other health care professionals. All roles and professions are invited to contribute. Participation is voluntary, and all survey responses are anonymous. The survey will be open until February 3, 2024. Upon completion of the survey, you will gain access to health care provider resources related to dementia care.
