Coming Soon: 1st Semi-Annual ’26 Preliminary CMID Reports

The SA1 ’26 Preliminary CMID Report includes MDS assessments from 10/01/2025 to 03/31/2026 that are transmitted to CMS/iQIES by 04/09/2026.

This report will be emailed to the address authorized by your facility as of 04/09/2026.

Please transmit assessments or corrections by end of business day on April 9, 2026. Submissions will be included in the SA1 ’26 Final CMID Report in the week of May 11, 2026.

If you have questions about defaults, excluded residents, corrections, modifications, an inactivation, or need a copy of the CMID report please contact [email protected]

Confidentiality

When emailing the [email protected], please do not list the resident name or other identifying information unless you are using a Secure Email. Instead, please include only the “Resident ID” number found in the column on the right of the resident name on the CMID report.  We are able to review your inquiries as we have access to your CMID reports.  If we need to send you detailed information about a specific resident(s), we will send the information via Secure Email and you will need to log-in with a password to retrieve it.
