Memory Care Certification Application Updated

DSHS has updated the memory care certification application; this update clarifies the need for an exemption due to a facility’s enforcement actions, as well as directions on where to submit the application.

If you have already submitted the memory care certification application in its previous form, there is no need to update and resubmit; the previous version will suffice.
