As part of the pilot cohort, you play a key role in helping to shape the future of the LPN Apprenticeship program. The questions in this assessment will help us gain a better understanding of the diverse needs of our apprentices so that we can provide greater supports and resources to you and to future cohorts.

Your individual responses will remain confidential and may be used in summary form to request additional funding or support for the program.
