DSHS Updates Transfer/Discharge Document Within Three Weeks of Original Publishing

DSHS recently updated the new Notice of Transfer or Discharge to include the physical address from which the resident is transferring or discharging. This update is due to the fact that there are many assisted livings and adult family homes with the same or similar names; the address will differentiate between them for clarity.
