Survey on Surveys for LTC Facilities
We are looking for your feedback on the survey and complaint investigations processes. If you have experienced a recent survey or inspection (annual, follow-up, or complaint) please tell us about the experience using our secure, online survey on surveys. You can complete this survey ANYTIME following an inspection, so be sure to bookmark the survey location on our website so you can return later.
The feedback we receive will be used to track and trend regional citation areas of focus as well as topics warranting educational development. For those individuals interested in sharing additional details, the facility’s Statement of Deficiencies report/2567 can be uploaded into the survey. WHCA staff will use specific examples (minus facility name/identifiers) when addressing issues with RCS to emphasize survey and communication improvement methods.
